Welcoming Our Newest Practice Coach/Trainer, Candy Magana!

The CEPC is pleased to announce that we have a new member in our practice coaching and training team: Ms. Candy Magana! Candy comes to the CEPC from the San Francisco Health Plan (SFHP). During her time as a Project Manager at SFHP, she co-managed the 10 Building Blocks Practice Coaching Program, served as a practice coach, and managed the development and implementation of an $8M block funding grant program for the SFHP network.

Prior to relocating back to the Bay Area, Candy was the Quality Improvement Program Manager at New York State’s primary care association—the Community Health Care Association of New York State (CHCANYS). In this position, Candy managed two state-wide quality improvement initiatives focused on hypertension and pediatric obesity. She trained and coached health center teams on quality improvement fundamentals, provided technical assistance, and managed the administration of all program activities. She also served as a Master Trainer for Stanford’s Chronic Disease Self-Management Program (CDSMP).

Candy has also served as a Training Coordinator at the National Minority AIDS Council in Washington, DC, and as a Research Assistant at Abt Associates Inc. in Bethesda, MD. She holds a Master of Public Administration in health policy and management from the Robert F. Wagner Graduate School of Public Service at New York University, and a Bachelor of Arts degree in public health studies from the Johns Hopkins University.
