Candy Magaña and Claire Richardson of the Center for Excellence in Primary Care (CEPC) attended the 2nd Annual ALL HEART/PHASE Convening on March 5, 2014, in Costa Mesa, CA. As part of the Northern California PHASE Support Team that will provide training and technical assistance to grantees in areas such as health coaching, panel management, and team-based care, CEPC had the opportunity to meet Northern California PHASE grantees in person, network with partners and field questions, and to learn all the exciting work being done in California through Kaiser Community Benefit Foundation’s ALLHEART/PHASE efforts.
