In 2014, the UCSF Center for Excellence in Primary Care (CEPC), with funding from the Blue Shield of California Foundation, completed a comprehensive literature review outlining strategies Community Health Centers (CHCs) use to integrate into the medical neighborhood in multiple domains. Forty-two percent of adults with health issues report problems with the coordination of their care in the United States, and challenges with care coordination are magnified in safety net settings.
To push successful examples of innovations and strategies for medical neighborhood integration to the larger community of safety net providers and organizations, CEPC has partnered with the Center for Care Innovations (CCI), to disseminate the results of the review. This partnership will allow the spread of best practices in an innovative fashion, with actionable, informative, practical information for CHCs and other safety net settings.
