Family Medicine for America's Health is a collaboration between eight family medicine organizations in the United States, with the mission to drive the continued improvement of the U.S. health care system and demonstrate the true value of primary care. The overall goal of this initiative is to develop a strategy and communication plan to continue the transformation of family medicine to meet the needs of the American public by achieving the Triple Aim of better care, better health, and lower cost.
There are seven tactic teams working together to achieve this goal, one of them being engagement with patients and other primary care professions. The Engagement Team, in collaboration with UCSF Center for Excellence in Primary Care, has been working with many different primary care practice around the country to identify how they systematically engaged their patients in the design and delivery of care. There will be 13 case studies generated to describe useful practices that will be shared broadly with the family medicine community in 2018.
