Join the UCSF Center for Excellence in Primary Care and CareOregon at our next Practice Coaching for Primary Care Transformation (PCPCT) Training, to be hosted in Portland, Oregon! The training will take place from Tuesday, October 22, through Friday, October 25, starting at 8:30 AM and ending at 5:00 PM all four days.
PCPCT is a multi-faceted training program designed for leaders at all levels who have a formal or informal coaching role in primary care transformation efforts. The course is an orientation to, and comprehensive review of, the science of change in primary care practice that equips participants with practical tools and coaching skills necessary to engage and support teams as they evolve into high-performing Patient-Centered Medical Homes.
This workshop is designed for practice coaches who work with primary care practices to assist with transformation efforts towards high performing Patient Centered Medical Homes.
This course includes four full days of classroom training, a pre-training assessment, curriculum materials, a site visit to a high-performing primary care practice, and optional post-training mentoring support. This robust training approach educates participants on the fundamental content of practice coaching while also providing them with the resources needed to empower clinic teams in utilizing quality improvement methods for sustainable change.
To ensure a comprehensive learning experience, the number of participants per training is limited and available on a first-come, first-served basis. Additional registration will be shared shortly.
