Practice Transformation Trainer (Analyst III)

Primary care transformation requires the implementation of essential components of high performing primary care, such as team-based care and data-driven improvement. The CEPC calls these the Building Blocks of High Performing Primary Care. Drawn from the experiences of high performing primary care practices, the building blocks provide a roadmap to improvements in patient experience, population health, and the sustainability of primary care. The CEPC currently assists primary care health centers, both within the safety net of San Francisco and more broadly across the country, as they integrate these Building Blocks into their practices through training to clinics seeking to implement team based care, panel management, and complex care management. In 2016 and 2017, CEPC has the opportunity to work with the Pacific Business Group on Health (PBGH) to design, develop, and implement a training program geared at transforming primary care and multi-specialty practices so that they deliver high quality, high value care. This is a two-year training program for practice coaches, project teams, and key leaders within provider organizations (POs) across the country. Incumbent may also work on similar related projects.

As a Practice Transformation Trainer (PTT), the individual will design and deliver highly interactive, skills-based trainings to medical professionals, paraprofessionals, and staff. These trainings are designed to help staff and clinicians learn new skills in advanced models of primary care and quality improvement. Training may be delivered to existing staff at practice coaching sites, to students and faculty at colleges and universities, and regionally and nationally to organizations seeking to implement new models of care.

Please note: This is a full-time contract position with a proposed start date of February 1, 2016, and an end date of one year after hire. If additional long-term funding is secured, the position may develop into a career position.
